إننا نجري بحثًا حول انعدام الأمن الغذائي من خلال سلسلة من الاستبيانات القصيرة لفهم النظام الغذائي في منطقتك بشكل أفضل.

Consent

We are conducting research on food insecurity through a series of short surveys to better understand the food system in your area.

Funding for this research is provided by the Defense Advanced Research Projects Agency [DARPA] of the United States Department of Defense.

The purpose of this research is to find out what people in your area think about the challenges and problems associated with getting enough food for themselves and their families.

We are asking you whether you will allow us to invite you to participate in a study about the food system in Babil. If you agree, we will ask you to complete a short survey that should take you less than 10 minutes to complete. You may be asked to complete up to two (2) more surveys in the future. The survey will be conducted over the course of 2 months.

Please understand that your participation is voluntary and you are not being forced to take part in this study. The choice of whether to participate or not, is yours alone. If you choose not to take part, you will not be affected in any way whatsoever. If you agree to participate, you may stop participating in the research at any time and tell me that you don’t want to go continue. If you do this, there will be no penalties and you will not be prejudiced in any way.

All identifying information will be kept in a locked file cabinet and will not be available to others and will be kept confidential to the extent possible by law. Your data will be transferred to the United States of America for processing and only be reported in the aggregate and will not be linked to any identifiable personal information in any way. The records from your participation may be reviewed by people responsible for making sure that research is done properly, including members of the ethics committee at the Human Sciences Research Council. [All of these people are required to keep your identity confidential.] Otherwise, records that identify you will be available only to people working on the study, unless you give permission for other people to see the records.

Your answers will be stored electronically in a secure environment and used for research or academic purposes now or at a later date in ways that will not reveal who you are. All future users of the stored data are required to apply for further Research Ethics Committee review and approval for secondary use of the stored data. The research staff will protect your data from disclosure to people not connected with the study. Gallup’s Principal Investigator will review all data to ensure that it does not contain any information that could potentially be used to directly or indirectly identify you. Any such directly or indirectly personally identifiable information will be stored in a designated secured recruitment/recontact datasheet that will only be used by Gallup/GeoPoll to contact you for the purposes of this study. However, complete confidentiality cannot be guaranteed because representatives of the U. S. Army Human Research Protections Office may inspect the records obtained in this study to ensure compliance with laws and regulations covering research with human subjects. In addition, data from your participation that is not identified [i.e. anonymized] may be shared with other researchers. The results of this research study may be presented at scientific or professional meetings or published in scientific journals and on-line outlets.

At the present time, we do not see any risk of harm from your participation. The risks associated with participation in this study are no greater than those encountered in daily life.

Your participation in this research will not cost you anything

There are no immediate benefits to you from participating in this study. However, this study will be extremely helpful to us in that we hope will promote understanding of food insecurity in Babil.

If you would like to receive feedback on our study, we will record your phone number on a separate link and can send you the results of the study when it is completed, within 12 months.
